She didn't push on her back for the entire four hours. As you can see a lot of the time she was on her hands and knees which is a great position. She was so exhausted after four hours she just wanted to lie down on her back in bed. I think what you can see them doing which I think works is push on your hands and knees for a bit, then when that's tiring you can lay down and push for a bit. Then she moved back to her hands and knees. And so on. That said, I think if her birth helpers had suggested they give her lots of support to get in a squat that position might have been just enough to get over that last hurdle of getting the head out she was having such trouble with in other positions. As you can see the head was pretty much crowning throughout the last half hour or so of pushing, so that last little "umph" might have been enough to pop it out. On the flip said, a nice slow crowning helps to stretch her tissues and prevent tearing. Her tissue was certainly given PLENTY of time to stretch around the baby's head.

I can't help but wonder about her being on her back, which is generally the worst position, favored by OBs but not most homebirth midwives, with the exception of that manuever to help free stuck shoulders. Was that what was happening? That she was on her back to try to free up stuck shoulders? What's the backstory to her attempting to push flat on her back?

Note, this mommy pushed about four hours. A fine example of how capable our bodies and minds are of delivering a baby naturally, in the quiet and comfort of home, even if the delivery seems to be taking a long time.
